The Mason FOREVER Foundation is a 501(c)(3) non-profit organization that promotes youth sports and students pursuing a degree in the medical/health/fitness fields. The foundation was established in 2020 in honor of Mason Sawyer Monteleone, a Junior on the Deans List at Florida State University. He was majoring in Interdisciplinary Medical Sciences when he tragically died in a car accident on August 28th, 2020. Sports, physical health, mental health and education were core to his being. Mason played youth football for the Lewiston Porter Youth Football Club and travel soccer for the Niagara Pioneer Soccer Club. He also participated in soccer, basketball, wrestling and track while in middle school at Lewiston-Porter. He attended Canisius High School where he participated in soccer, rugby and wrestling. He was also a member of the Catholic NY State Champion Canisius High School football team. He enjoyed snowboarding, skiing, and jet skiing. The Mason FOREVER Foundation was established to support other young people that share the same passions as Mason. We would like to thank everyone that has already donated, your generous support keeps Mason alive in our hearts and memories by promoting the ideals that embodied him.

We recognize the extraordinary financial need of medical students and all students. The Mason FOREVER Foundation grants scholarships to like-minded learners who are studying in the health or fitness fields. If you are passionate about helping people and in making a difference in their lives, working in healthcare/fitness are some of the most worthwhile career choices there are. The Foundation offers scholarship opportunities to students pursuing a career in these fields.
Scholarship opportunities are available to students currently enrolled in any health/fitness program at an accredited college/university in the United States. To be eligible the applicant must have a minimum grade point average of 3.5 on a 4.0 scale. All application materials must be submitted in a single envelope mailed Jan - July of current year, to Mason FOREVER Foundation Scholarship, 190 Oliver Street SUITE 300, North Tonawanda, NY 14120 and must be contain the following:
- curriculum vitae
- current program and institution
- degrees earned, name of institution(s) and year(s) of graduation
- employment experience
- 3 letters of recommendation regarding academic status, career commitment, fitness/health/sports commitment
- narrative that includes (subtitles for each of the following 4 areas) personal background, sports/fitness/health experiences, community service experiences, professional goals
Scholarships will be awarded every year on August 16th.
